Your privacy

We designed this program to collect as little personal information as possible, for as short a time as possible. Here’s exactly what happens with your information.

What we collect

• The name you give us for the shipping label
• Your Oregon mailing address

We do not collect a recipient email address or phone number.

We never ask for ID, insurance information, a prescription, or why you need naloxone.

You must be age 13 or older to submit the online order form. We do not knowingly collect personal information online from children under age 13.

How your information is protected

• Only a small number of authorized program staff can view your order in our systems, behind a secure login.
• We send the minimum information needed to approved service providers to confirm the mailing address, create the shipping label, and deliver the package.
• After an order is delivered or cancelled, we keep direct shipping information for a 30-day fulfillment-support period. After that period, we remove the recipient name, full address, tracking details, and shipping-provider identifiers from the active application, and remove the stored shipping-label file through our routine privacy cleanup.
• Separately, we use limited, privacy-protected information for 60 days after a request to support authorized order review. It does not contain a readable name or address, is not included in reports or exports, and is never used to automatically reject an order. It is not used after that review period and is removed through routine privacy cleanup.

The optional survey

If you receive a QR code or link inviting you to share how your naloxone was used, it’s completely optional and anonymous. Survey responses are stored separately from your order and are never connected to your name, address, or order number.

Because we do not collect recipient email addresses, we do not keep an email list for survey invitations.

What the State of Oregon sees

Oregon Health Authority receives aggregate program information, including request and fulfillment dates, ZIP codes, counties and general areas served, numbers of orders, boxes and doses distributed, fulfillment outcomes, first-request or repeat-request classifications, and anonymous survey totals. OHA never receives your name or full address. Information shared more broadly is grouped across geography and time, with small results suppressed when they could identify someone.
